Hazel V. Small
April 5, 1917 - February 25, 2005

     Hazel V. (Cook) Small, 87, of Forestville, formerly of Irving and Perrysburg, died Feb. 25, 2005 at Brooks Memorial Hospital in Dunkirk.
     Mrs. Small was a graduate of Gowanda High School and had been employed as a nursing aide at the former Lake Shore Intercommunity Hospital in Irving.
     She was a member of the Versailles United Methodist Church and former member of the Gowanda Free Methodist Church. Her interests included birds and wildlife.
     She was the wife of John H. Small; daughter of the late Burt and Caroline (Harvey) Cook; mother of Carol (Richard) Best of McKinney, TX, Coral (Robert) Dye of Cedar Rapids, IA, and Thomas (Sherry) Small of Forestville; and sister of the late Alice Goran, Edna Wallace, Florence Herkel and Harold Cook; also survived by 11 grandchildren, 20 great-grandchildren and several nieces and nephews.
     Memorials may be made to the Alzheimers Association of WNY, 2805 Wehrle Drive, Suite 6, Williamsville, NY 14221, or to the Forestville Fire Department Emergency and Rescue Squad, Chestnut Street, Forestville, NY 14062.
     Arrangements were by the Riles & Woolley Funeral Home, Forestville.
